State of Illinois school physical forms, dental forms and vision forms are available on the Illinois Department of Public Health Web site at the links below.

School physical exams/proof of immunizations are required when your child is entering kindergarten, 6th grade and 9th grade. 

Sports physical exams are required when your child will be participating in a sport at the middle or high school level. Sports physicals are valid for one year (365 days) and cannot expire at any time during the sport your child is involved. If you take your child for a sports physical during the summer, it will be valid for the duration of the school year.

School dental exams/waivers are required when your child enters kindergarten, 2nd and 6th grades.

Vision exams/waivers are required when your child enters kindergarten.
